Liberty ADH provides a safe environment during the day for seniors and younger adults with physical, cognitive, or behavioral impairments.
· Medical Care: Skilled nursing tasks, medication management, and monitoring of chronic conditions (e.g., blood pressure, diabetic management and education and much more).
· Social & Mental Stimulation: Planned activities such as arts and crafts, music, games, and gentle exercise, entertainment and community outings.
· Nutrition: A hot, nutritious breakfast, lunch and snack daily (we follow Child and Adult Food Program (CACFP)).
· Personal Care: Assistance with activities of daily living (ADLs) like grooming, eating, and mobility.
· Transportation: Liberty provides round-trip transport to and from the facility.
